Mental Health Connectors
The Mental Health Connectors strive to create an effortless transition from clinical care to services and organisations that offer individuals the essential support they need.

Mental Health Connectors from Better Pathways, Living Well Consortium, and Birmingham Mind work with Birmingham and Solihull Mental Health Foundation Trust (BSMHFT) to navigate people towards psychological therapies, improved physical health care, employment support, personalised and trauma-informed care, medicine management, and support for self-harm and coexisting substance use.
Participants will benefit from:
- Receiving accurate, wrap-around care which is quick to access
- Better care, resulting in improved recovery and independence
- Earlier detection, intervention, and prevention
- Being assigned to person-centred pathways, based on the individual’s needs
- Feeling more in control of their care, and of who is providing the support, when and why


We can help you with:
- Finding and joining a local support or community groups
- Anxiety and depression management techniques and resources
- Accessing community resources
- Support accessing specialist advice, for example: employment, housing, bereavement, money or relationships
- Locating therapy and counselling services

About Mental Health Connectors
We work within the Community Mental Health and Wellbeing Service, we are part of the Neighbourhood Mental Health Team which includes GP’s, nurses and other mental health practitioners.
We can help you connect with services available in your local community to receive support with issues affecting your mental health and wellbeing.What sessions look like
With the support of a Mental Health Connector, you will have a safe space to speak about any worries or concerns. Your Mental Health Connector will provide proactive support to create a plan to overcome these concerns.
This can involve identifying relevant services and organisations that can support and help to manage your mental health and wellbeing.Where are the appointments held?
We are adaptable and can offer face-to-face sessions within the community, as well as telephone appointments.
Who is eligible for the service?
Available to those aged 18+
Not currently under secondary mental health care, including Community Mental Health Teams or home treatment.How to access the service
Contact your GP surgery and ask to be booked in with a Mental Health Practitioner. Ask the Practitioner if the Neighbourhood Mental Health Team is embedded in your GP surgery. If this is the case, you can request a referral to the Mental Health Connector service.
If the Neighbourhood Mental Health Team is not embedded in your GP surgery, ask for an alternative service.
